Student Registration Form

          This form is the Emergency Medical Authorization Card.  This form needs to be filled when the student enters the district.

           The form allows or not allows the school district to use student photos for public relations and school events.

  
   
Free-Reduced Lunch Application

              Fill out this form to see if you qualify for the Free and Reduced Lunch program.  Submit the completed form to the building office or to Food Service.

  
   
Acceptable Use and Internet Safety Form

           This form is to gain access to the Fostoria Community School Network.  Students and parent need to agree to the school policy by signing the forms and submitting them to the buildings.
