Final Forms

Fostoria City Schools families. This is a reminder that Fostoria City Schools has partnered with Final Forms, an online forms and data management service for the 2019-2020 school year. You will now be required to complete and sign all annual forms for your students online. Directions on how to get started using Final Forms were mailed home in May. We are asking that all families complete these steps as soon as possible to help ensure their students are registered for the upcoming school year. Fostoria City Schools is partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign all annual enrollment and athletic participation forms for your students online – it is simple enough you can do it right from your smartphone. FinalForms saves data from year-to-year and season-to-season, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. Once you have completed your registration online, you will simply be asked to review and update your information at the beginning of each school year to verify the information is current and sign the forms.

Dear Parents/Guardians of Fostoria City Schools,

We believe it is important to communicate with our families.  One way we do that is with our instant message system. We are using a company called School Messenger. 

The phone numbers and emails that are used initially are the contacts listed in our student information system (DASL). If you would like to be able to change how you are contacted, we will need a current email address in our system. Please contact your child's school to add personal email address(es) to your child's profile in the District's Student Information System.

Once your address is in you will be able to go to and create an account that will allow you to add numbers and email addresses to your profile. Note: Changes made through the School Messenger application do not inform your child’s school of the changes.  Parents/Guardians should also inform the school of any changes to phone numbers and/or email addresses.

School Messenger provides you with the opportunity to “Opt-In” if you wish to receive school/district messages via text by texting“Yes” to our district’s short code number, 67587.  You can also “Opt Out” of receiving text messages at any time by simply replying to one of our messages with “Stop.”   

School Messenger is compliant with the Student Privacy Pledge, so you can rest assured that your information is safe and will never be given or sold to anyone.

Title IX

What is Title IX?

No person in the United States shall, on the basis of sex, be excluded from participation in, be denied the benefits of, or be subject to discrimination under any educational programs or activity receiving federal financial assistance. 
-- From the preamble to Title IX of the Education Amendments of 1972

Title IX of the Education Amendments of 1972 prohibits sex discrimination in educational programs and activities. All public and private schools, school districts, colleges and universities receiving federal funds must comply with Title IX.

The regulations implementing Title IX are enforced by the U.S. Department of Education Office for Civil Rights and prohibit discrimination, exclusion, denial, limitation, or separation based on gender. Intended to end sex discrimination in all areas of education.


The District does not discriminate on the basis of sex in its education program or activities, including admission and employment, and is required by Title IX and its implementing regulations not to discriminate in such a manner. Inquiries about the applicability of Title IX and its implementing regulations to the District may be referred to the District’s Title IX Coordinator, to the Assistant Secretary for the U.S. Department of Education’s Office for Civil Rights, or both.

Reports and Complaints of Harassing Conduct

The Board is committed to promptly and equitably resolving student and employee complaints alleging Sexual Harassment. Members of the School District community, (students and Board employees, as well as, Board members, agents, volunteers, contractors, and all other third parties) are encouraged to promptly report incidents of harassing conduct to an administrator, supervisor or other School District official so that the Board may address the conduct promptly. Any administrator, supervisor, or other District official who receives such a complaint shall promptly notify the District Title IX Coordinator. Complaints can be received in person, by mail, by telephone or by electronic mail.

Investigation and Complaint Procedure

The District has adopted a grievance procedure to provide for the prompt and equitable resolution of student and employee complaints alleging any action that would be prohibited by Title IX and its implementing regulations. This procedure describes how to report or file a complaint of sex discrimination, how to report or file a complaint of sexual harassment, and how the District will respond.
