We are in the process of planning for the upcoming 24-25 school year and we are so excited that your child will be joining us! Kindergarten screening will be held May 3, 2024. Below are the links to register your child in Final Forms and to schedule an appointment. In order to make an appointment you must complete Final Forms PRIOR to signing up. Your child must be 5 years old by August 1st

Below is a checklist of the items your child will need in order to attend Kindergarten:

Before Kindergarten Screening:
-Birth Certificate
-Parent Identification
-Proof of Residency
-Final Forms (Final Forms will open April 15th)
-Custody Papers if applicable
-Immunization Records so we can verify what immunizations your child still needs.

Before they start the first day of school:
-Physical (if a physical was completed for PK, you will not need to complete this again).
-Kindergarten Dental Report
-Required Kindergarten shots (5 DTAP, 4 Polio, 2 Chicken Pox, 2 MMR, 3 Hepatitis B)
