Final Forms
Fostoria City Schools families. This is a reminder that Fostoria City Schools has partnered with Final Forms, an online forms and data management service for the 2019-2020 school year. You will now be required to complete and sign all annual forms for your students online. Directions on how to get started using Final Forms were mailed home in May. We are asking that all families complete these steps as soon as possible to help ensure their students are registered for the upcoming school year. Those who would like help or need access to a computer are asked to please call 419-435-8163 to set up an appointment. 2020-2021 Enrollment
Fostoria City Schools is partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign all annual enrollment and athletic participation forms for your students online – it is simple enough you can do it right from your smartphone. FinalForms saves data from year-to-year and season-to-season, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. Once you have completed your registration online, you will simply be asked to review and update your information at the beginning of each school year to verify the information is current and sign the forms.

Dear Parents/Guardians of Fostoria City Schools,
We believe it is important to communicate with our families. One way we do that is with our instant message system. We are using a company called School Messenger.
The phone numbers and emails that are used initially are the contacts listed in our student information system (DASL). If you would like to be able to change how you are contacted, we will need a current email address in our system. Please contact your child's school to add personal email address(es) to your child's profile in the District's Student Information System.

Once your address is in you will be able to go to https://go.schoolmessenger.com/#/home and create an account that will allow you to add numbers and email addresses to your profile. Note: Changes made through the School Messenger application do not inform your child’s school of the changes. Parents/Guardians should also inform the school of any changes to phone numbers and/or email addresses.
School Messenger provides you with the opportunity to “Opt-In” if you wish to receive school/district messages via text by texting“Yes” to our district’s short code number, 67587. You can also “Opt Out” of receiving text messages at any time by simply replying to one of our messages with “Stop.”
School Messenger is compliant with the Student Privacy Pledge, so you can rest assured that your information is safe and will never be given or sold to anyone.
